The First Pill: Mifepristone

You will take mifepristone at your appointment or be given it to take at home.

  • Most people feel very little after mifepristone. Mild nausea or light spotting is possible but most people feel essentially normal.

24 to 48 Hours Later: Misoprostol

This is the medication that starts the active process. Take it exactly as your provider instructs.

  • For buccal administration place tablets between your cheek and gum and allow them to dissolve for the full 30 minutes before swallowing residue.

What Happens After Misoprostol

  • Cramping typically begins within 1 to 2 hours of administration.
  • Heavy bleeding follows shortly after cramping begins.
  • The peak cramping and bleeding phase lasts approximately 2 to 4 hours for most patients.
  • The gestational sac and pregnancy tissue pass during this phase, often accompanied by a distinct shift from peak cramping to progressively lighter contractions.

What to Prepare Before Taking Misoprostol

Have all of this ready before you take the second medication so you do not need to get up during the active phase.

The Pain: What to Realistically Expect

Most patients rate abortion pill pain between 6 and 8 out of 10 during the peak phase. It is real and significant. Being prepared for it rather than surprised by it makes it more manageable.

  • It feels like intense waves of lower abdominal cramping and pressure that come and go.
  • Lower back pain alongside the abdominal cramping is common.

The Bleeding: What Is Normal

Other Side Effects to Expect

  • Nausea and sometimes vomiting from misoprostol’s prostaglandin effects.
  • Diarrhea within 1 to 2 hours of misoprostol. Stay hydrated.
  • Chills and low grade fever in the hours immediately following misoprostol. This is a normal medication side effect not a sign of infection.
  • Dizziness, particularly when standing during heavy bleeding.

What Is Not Normal: Warning Signs

Know these before tomorrow so you can act quickly if needed.

  • Soaking more than 2 thick pads per hour for 2 consecutive hours. Go to the emergency room immediately.
  • Fever above 100.4 degrees lasting more than 4 hours after the initial misoprostol side effects have passed.
  • Sharp one sided pelvic pain with dizziness or shoulder pain requiring emergency evaluation.
  • No bleeding within 24 hours of misoprostol.

The Days Following

  • Moderate bleeding continues for approximately 1 to 2 weeks.
