While clinical guidelines have become more flexible in 2026, there are several practical and medical reasons to wait a short period before switching back to tampons or menstrual cups.
1. Tracking Your Bleeding
The most important reason to use pads during the initial process is to accurately monitor your blood loss.

The Safety Metric: We use “pads per hour” as the primary way to determine if bleeding is within a safe range.

The Limit: If you soak through two heavy-flow maxi pads per hour for two consecutive hours, it is a sign that you need to contact our clinical team immediately. It is much harder to measure this volume accurately with tampons or a cup.
2. Passing Clots
During a medication abortion, your body needs to expel pregnancy tissue and blood clots.

Obstruction: Tampons can physically block the passage of larger clots, which may lead to increased cramping as the uterus works harder to push them past the obstruction.

Observation: It is helpful to see the size of the clots you are passing (anything larger than a lemon is a sign to call us).
3. Reducing Infection Risk
While the risk is low, your cervix remains slightly more open during and immediately after the abortion process.

The 1–2 Week Rule: To minimize the chance of bacteria entering the uterus, many providers suggest avoiding putting anything inside the vagina (including tampons and menstrual cups) for about one to two weeks, or until your heaviest bleeding has subsided into light spotting.
4. When Can You Switch?
Once your bleeding has significantly lightened—typically 3 to 5 days after taking the second medication—you can usually return to whatever menstrual product you find most comfortable.

Listen to Your Body: If using a tampon causes increased cramping or discomfort, your body may still need more time to heal. Switch back to pads for another day or two.

Hygiene: Ensure you are changing tampons or cups frequently (every 4–6 hours) to keep the area clean as your internal tissues recover.
