Primary Care in Des Plaines That Connects What Other Practices Treat Separately
Women’s health rarely fits neatly into a single specialty. A gynecologist handles your reproductive system. A general practitioner addresses your blood pressure. A mental health provider manages your anxiety. But no one is looking at how all three interact how chronic stress affects your cycle, how your thyroid function connects to your mood, how the medication you’ve been on for years is quietly compounding a hormonal imbalance that’s never been identified.
That is the gap Serenity Choice Health was built to close. Our licensed Illinois providers approach women’s primary care as a connected discipline, not a collection of isolated appointments. When you establish care with us, your cardiovascular health, your hormonal patterns, your reproductive goals, your mental wellness, and your chronic conditions are all part of one clinical picture reviewed together, managed consistently, and followed over time by a provider who actually knows you.

Our Women's Primary Care Services
Comprehensive Wellness Exams
Your annual exam should build on last year’s not restart from zero. We review your physical, hormonal, reproductive, and mental health together, so what we find this visit actually informs the next one.
Thyroid & Metabolic Health Management
Fatigue, weight shifts, and mood changes get blamed on stress far more often than they get tested properly. We run the right panels, read them against your symptoms, and treat what we actually find not what’s easiest to explain away.
PCOS Evaluation & Long-Term Management
PCOS shows up as more than an irregular period it drives insulin resistance, mood instability, skin changes, and fertility concerns. We manage the full picture, not just the symptom that finally got you a referral.
Perimenopause & Menopause Care
Brain fog, broken sleep, mood shifts, and hot flashes are not things to simply endure. We evaluate your hormonal picture completely and build a plan around your preferences and risk profile not a standard protocol that ignores both.
Cardiovascular & Blood Pressure Management
Heart disease kills more women than any other condition and consistently gets caught late because it presents differently in women. We screen proactively, manage hypertension with real clinical oversight, and flag risk before it becomes a crisis.
Preventive Screenings Tailored to Your Risk Profile
Cervical, breast, bone density, diabetes, and STI screenings are ordered around your actual age, history, and risk factors not a generic schedule built for a patient who isn’t you.
Common Situations That Bring Patients to Our Clinic
- Establishing consistent primary care after years of episodic or fragmented appointments
- Fatigue, weight changes, mood disruption, or cycle irregularity that hasn't been adequately explained elsewhere
- Managing PCOS, thyroid conditions, or autoimmune disorders alongside reproductive health concerns
- Perimenopause or menopause symptoms that are affecting daily quality of life
- Wanting preventive screenings handled by a provider who knows their full history
- Mental health concerns that need to be part of primary care, not an entirely separate system to navigate
- Cardiovascular monitoring for women with family history or emerging risk factors
- Medication management that involves actual clinical review rather than routine refills
- Coordinating reproductive health planning and contraceptive care under the same roof as primary care
- A desire for a provider relationship that carries real continuity where something said at one appointment is remembered at the next
What to Expect During Your Visit?
Establishing care at Serenity Choice Health begins with building a complete picture, not processing a new patient form:
- A thorough intake covering your health history, family history, current medications, past diagnoses, and anything that’s been left unresolved or unexplained
- Direct conversation about what brought you in today and what you’ve been managing without adequate clinical support
- A physical examination and baseline lab work ordered around your specific age, risk profile, and stated concerns
- Plain-language explanations of every finding no jargon that leaves you Googling in the parking lot
- A written care plan with clear priorities, follow-up timing, and referral coordination where specialist input is needed
- Telehealth options established for future visits that don’t require an in-office exam
Your first visit ends with a clear understanding of your health baseline, what we’re watching, what we’re treating, and how we’ll stay in contact between appointments.
How We Support Your Health Between Appointments?
Establishing care is the beginning, not the goal. We maintain your health over time through:
- Scheduled annual and interval wellness visits structured around what your health actually requires at each stage
- Lab monitoring cadence appropriate to your chronic conditions, hormonal health, and preventive needs
- Medication management with active clinical oversight between refill cycles
- Telehealth availability for follow-up visits, lab result reviews, and between-visit clinical questions
- Specialist coordination so referrals don’t become dead ends we stay in the loop and integrate what the specialist finds back into your overall care
- Ongoing documentation of your health changes over time so patterns are visible, not pieced together from memory at each visit

Annual wellness exams, preventive screenings, and most primary care visits are covered under major insurance plans, and many preventive services for women are available at zero out-of-pocket cost under the Affordable Care Act. Our billing team verifies your specific benefits before you arrive so cost is never an ambush you know what to expect financially before the appointment begins.
We welcome uninsured patients, accept Medicaid, and work with patients whose coverage situation is complicated or in transition. If cost is a concern, we will tell you plainly what assistance programs you may qualify for and what your realistic options are before you decide whether to book.
The Most Question We Got so Far
Still have questions? Reach out we’re just a message away.
I've been told my labs are normal but I still don't feel well. Is there something being missed?
Possibly, yes. “Within range” on a standard lab panel and “functioning optimally” are not the same thing and standard panels don’t always test what matters most for the symptoms you’re describing. We look at your results in the context of your full clinical picture, your symptoms, and what’s changed over time, not just whether a number clears a threshold.
Can I see the same provider every visit instead of rotating through whoever is available?
Yes. Consistent provider access is a core part of how we operate. A care relationship only works if the person treating you knows your history well enough to notice when something has changed.
Do you manage both my general primary care and my reproductive health, or do I still need a separate ob-gyn?
We handle a wide range of reproductive health concerns within primary care cycle management, hormonal evaluation, contraceptive counseling, perimenopause care, and more. When your situation requires specialist-level ob-gyn or maternal-fetal care, we coordinate that referral and stay involved throughout rather than handing you off completely.
I'm in my 40s and things feel different but no one has explained why. Is this something you address?
Perimenopause often begins years before the last menstrual period and produces a wide range of symptoms that frequently go unattributed or undertreated. Hormonal shifts in the decade before menopause are real, clinically significant, and very addressable. We evaluate what’s happening hormonally and discuss what approaches fit your preferences and health history.
